An experienced and reliable home care Occupational Therapist is needed for home visits in Westchester County. Key responsibilities include:

• Assessing patient needs and developing individualized care plans based on physician prescriptions.
• Providing therapy treatment, documenting patient responses, and modifying plans as necessary.
