Position Summary: The PT – Physical Therapist delivers direct rehabilitation care to patients based on physician referrals, adhering to relevant laws and standards.
Education, Experience, and Licensure Requirements:
Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT).
Current state registration/licensure, maintaining good standing.
Preferred one year of experience in post-acute care.
CPR certification required upon hire or during orientation.
Specific Job Requirements:
Engage in community health projects.
Proficient in Microsoft Office.
Coordinate with patients and families for care planning.
Demonstrate good body mechanics and independent decision-making.
Knowledge of post-acute care regulations.
Maintain confidentiality and professional relationships.
Essential Functions:
Implement treatment plans and set measurable goals.
Oversee care provided by support staff.
Ensure accurate documentation and use of therapy software.
Provide excellent customer service and assist in patient evacuations.
Demonstrate regular attendance and effective communication within a team.
