Licensed Practical Nurse

Job Description

We are seeking a Licensed Practical Nurse (LPN/LPT/LVN) to provide direct and indirect care to clients in their homes, following organizational policies. Responsibilities include:

• Delivering skilled nursing care per physician’s orders.
• Documenting health status and communicating with families and physicians.
• Administering medications and conducting health assessments.
• Providing health education and self-care techniques to clients.
• Ensuring compliance with state regulations and maintaining patient confidentiality.
• Training new employees and participating in staff meetings.

Qualifications include a nursing degree, medication administration experience, and a current state license. Home health care experience is preferred. Salary range is $20.00 – $22.00 per hour.
