We are looking for a compassionate Certified Occupational Therapy Assistant (COTA) to join our skilled nursing facility team in San Jose, CA.
Responsibilities:
Assist in evaluating patients’ needs and capabilities
Collaborate on individualized treatment plans
Provide hands-on therapy and interventions
Teach skills to enhance patient independence
Monitor progress and adjust treatment plans
Document patient outcomes accurately
Work with healthcare professionals for comprehensive care
Qualifications:
Experience in Skilled Nursing or Long-Term Care preferred
Certification as an occupational therapy assistant
Interest in geriatric care
Strong communication and teamwork skills
