Job Description

AW Health Care seeks an Occupational Therapist (OT) to provide intermittent therapy to patients in their homes, following physician orders and care plans under the Director of Therapy’s supervision.

• Responsibilities include improving patient disabilities, conducting evaluations, providing therapy, and consulting with physicians.
• Must prepare clinical notes, instruct patients and families, and participate in care planning.
• Qualifications: Graduate of an accredited OT program, current state license, and preferred two years of experience.
• Physical requirements include moderate lifting and the ability to engage in physical activity.
