We are seeking a PRN Physical Therapist (PT) to join our team in West Des Moines, IA. The ideal candidate will provide Physical Therapy services according to treatment plans and agency policies.

• Qualifications: Must be a licensed Physical Therapist in Iowa, with a preferred minimum of 2 years of clinical experience and prior home healthcare experience.
• Responsibilities: Include patient evaluations, treatment planning, progress documentation, family consultations, and coordination with healthcare staff.
