We are seeking a PRN Occupational Therapist – OTR/L to provide care for skilled nursing patients in Rifle, CO. This role involves evaluating patients’ functional status, developing treatment programs, and coordinating with an interdisciplinary team to enhance patient care experiences.

  • Assess patient needs and home environment
  • Develop and implement treatment plans
  • Supervise Occupational Therapy Assistants
  • Participate in discharge planning and case conferences

    • Qualifications:

  • ACOTE-accredited occupational therapy degree
  • 2+ years of experience, preferably in home health
  • Current state licensure

    • Salary range: $60.00 – $95.00/hour.
