Adara Home Health seeks an RN Case Manager to provide direct care and oversee home health services for clients. This role involves collaborating with clients, families, and healthcare teams to create goal-centered care plans, ensuring quality and financial considerations are met. The position offers a flexible schedule, primarily Monday to Friday, with an on-call rotation, covering areas like Cold Spring and St. Cloud.
