Job Description

Seeking a part-time Physical Therapist (20 hours/week) to provide home-based care and rehabilitation therapy for patients recovering from health events. Responsibilities include:

• Collaborating with healthcare teams to develop and implement patient-specific care plans.
• Monitoring and documenting patient progress.
• Supervising aides and therapy assistants.
• Completing OASIS assessments and maintaining continuity of care.

Requirements include a current NY Physical Therapist license, a Master’s or Doctorate in Physical Therapy, and preferably one year of experience.
