Social Worker – IP Hospice/Palliative Care

Job Summary: The Hospice/Palliative Social Worker provides consultative services in a hospital setting, focusing on enhancing the quality of life for patients with chronic or serious illnesses. This role involves a multidisciplinary approach to care, emphasizing symptom management, decision-making, and future care planning based on patient values. The social worker will collaborate with an interdisciplinary team and utilize crisis counseling skills to support patients and families.

Qualifications:

• Master’s degree in Social Work and current New Mexico licensure (LISW or LCSW) with five years of relevant experience, or LMSW with ten years of experience.
• Experience in case management; certification in Hospice/Palliative Social Work preferred.
