- Conducting bio-psycho-social-spiritual assessments.
- Providing individual and family counseling.
- Referring families to community resources.
- Maintaining accurate patient documentation.
- Participating in care planning and team meetings.
- Offering bereavement care and group activities.
Qualifications include a Master’s in Social Work, at least one year of healthcare experience, and relevant state licensure. Knowledge of community resources and psychosocial issues related to terminal illness is essential.
