Hospice Nurse

Job Description

We are seeking a Hospice RN Case Manager to provide in-home care for patients in the northern suburbs of Chicago. The ideal candidate will have experience in hospice care and be comfortable visiting patients at home.

• Responsibilities: Deliver direct patient care, manage cases, perform assessments, and collaborate with doctors on medication and health monitoring.
• Requirements: Illinois RN license, at least 1 year of hospice experience, reliable transportation, and strong communication skills.
• Compensation: Annual salary ranges from $85,000 to $95,000, with benefits including health insurance, 401(k), and paid time off.
