LPN/LVN – Licensed Practical/Vocational Nurse

Job Description

Job Summary: A Licensed Practical/Vocational Nurse provides skilled nursing care to patients in their homes, following physician orders and under the supervision of a Registered Nurse.

Responsibilities:

• Adhere to policies and implement nursing care plans.
• Provide treatments and preventive procedures.
• Monitor patient conditions and report changes.
• Educate patients and families on care needs.
• Maintain accurate patient records and participate in audits.
• Attend meetings and take on-call duties as needed.

Qualifications: Must be a graduate of an approved nursing program, licensed, and preferably have one year of nursing experience. Must demonstrate initiative and independent judgment.

Working Environment: Involves working in both office and home settings, with physical demands including heavy lifting and extended periods of standing.
