Care Manager (Registered Nurse/RN)

Job Description

The RN Care Manager coordinates patient care and services, ensuring quality through a collaborative process. This role involves working with patients, families, care teams, payers, and external entities to facilitate safe transitions between care settings while adhering to laws and standards.

Education and Experience Requirements:

• Nursing diploma (associate’s or bachelor’s degree)
• Active RN license in good standing
• One year of clinical experience in a post-acute care setting preferred
• Prior case management experience preferred
• CCM or RN BC certification preferred
