Responsibilities:
- Understand infant-toddler development and utilize a parent coaching model.
- Participate in assessments, screenings, and home visits as part of a trans-disciplinary team.
- Collaborate with community providers for continuity of care.
- Coordinate Individual Family Service Plans for assigned caseloads.
Qualifications:
- Masters degree in Social Work or Counseling Psychology with relevant licensure.
- Two years of pediatric experience preferred.
- Effective communication skills and computer proficiency required.
- Valid drivers license and reliable transportation needed.
