Requirements:
1+ year of RN experience with a valid license
Current CPR/BLS Certification
Valid driver’s license and insurance
Comfortable with home visits
Preferred experience in Home Health, Hospice, Infusion, or Home Care
Responsibilities:
Develop and implement patient Plans of Care (POC)
Establish patient-specific goals and provide nursing services
Oversee quality, patient-centered care
Communicate patient status with the care team
Document care accurately and timely
