Hospice Registered Nurse – Community – FT

Job Description

We are seeking a Registered Nurse (RN) to provide compassionate hospice care to patients in various settings, including private homes and assisted living facilities. The RN will work as a Case Manager within an interdisciplinary team, conducting psych-social assessments and collaborating with healthcare professionals.
