Job Description

Position Summary: The Registered Nurse Navigator will facilitate patient navigation through the healthcare system post-discharge, supporting care coordination in outpatient settings. Responsibilities include serving as a clinical resource for scheduling coordinators, identifying appropriate services, and addressing discharge needs.

Minimum Qualifications:

• Current RN license in the state of employment or multistate RN license.
• AHA BLS certification by the end of orientation.
