Home Care Speech Therapist – PRN

Job Description

Purpose: Provide comprehensive Speech Therapy services in home settings to enhance functional abilities related to speech, language, cognitive communication, voice, swallowing, and fluency.

Responsibilities:

• Evaluate patients and create treatment plans.
• Document evaluations, progress, and discharge summaries.
• Collaborate with rehabilitation team members.
• Develop home programs and treatment protocols.
• Monitor patient progress and adjust treatment as needed.
• Assist with staff orientation and quality assurance.

Qualifications:

• Master’s degree in Speech Language Pathology.
• Certificate of Clinical Competence or eligibility.
• Effective communication skills and ability to work in a team.
• Current CPR certification and mandatory reporter training.
• Valid driver’s license and reliable transportation.

Working Conditions: Medium physical demands, flexibility in scheduling, and travel between locations required.
