RN Admission Coordinator, Acute Rehabilitation

Job Description

We are seeking an Admission Coordinator for the acute inpatient rehabilitation program. This role is vital for facilitating access to rehab services and managing communication among the rehab team, physicians, patients, and caregivers.

Responsibilities:

• Track and communicate patient referrals and statuses
• Engage with patients and families
• Build relationships with case management staff
• Conduct insurance verification and pre-certification
• Attend team meetings and communicate updates
• Assess patient charts for eligibility and needs
• Provide estimated length of stay and code patients

Requirements:

• RN license (Bachelor’s preferred)
• Experience in acute rehabilitation preferred
• Knowledge of payor requirements
• Proficient in Microsoft Office and EMR systems
• Strong interpersonal and communication skills
