Job Summary: Clinical Care Coordinator

This full-time position offers an hourly wage of $28.00-$30.00, based on experience and certification, along with benefits such as 401(k), health insurance, and paid time off.

Key Responsibilities:

• Implement and manage residents’ personal care and service plans.
• Perform assessments, manage resident files, and coordinate outside services.
• Supervise staff and assist with employee training and scheduling.
• Monitor medication and lab orders, and communicate with healthcare providers.
• Participate in on-call duties and quality assurance meetings.

Qualifications:

• Licensed Practical Nurse in Michigan preferred.
• Training in TB testing and medication administration.
• Effective communication skills and compassion for residents.
• Ability to pass background checks and CPR certification required.

Physical and emotional stability to assist residents with personal care is essential. Reports to the Resident Services Coordinator.
