MDS LPN Nurse

Job Description

We are seeking a full-time LPN MDS Nurse / Resident Assessment Coordinator (RAC) to join our team. This role offers a competitive pay rate of $35-$40/hour and includes benefits such as life insurance, medical, dental, vision, and a 401(k) with employer match.

Qualifications:

• Graduate of an accredited nursing school; valid LPN license.
• Preferred three years of experience in long-term care and familiarity with MDS/RAI processes.

Responsibilities:

• Complete MDS assessments and coordinate the RAI process.
• Ensure compliance with regulations and appropriate reimbursement for residents.
