An established hospice agency is seeking an experienced Hospice RN Case Manager to provide care in LTC facilities and homes in the Andover, Reading, Melrose, Everett, and surrounding areas. Candidates must have at least 1 year of experience with a Medicare-certified agency. The role involves direct patient care, coordination, and advocacy for patients and their families, with a focus on end-of-life issues.
