A full-time RN Case Manager position is available in Oklahoma City, OK, working with the Trauma Unit team from 8 a.m. to 4:30 p.m. Responsibilities include:

• Conducting comprehensive assessments of patients’ clinical, psychological, and financial needs.
• Coordinating timely transfers and discharge planning.
• Facilitating communication among patients, families, and healthcare professionals.
• Managing resource utilization to optimize clinical outcomes.
• Maintaining knowledge of regulations and community resources.
