Position Title: Nurse – Wellness Coordinator

Purpose: As an LPN, you will enhance client well-being in group home and community settings by coordinating healthcare and developing personalized care plans. You will mentor staff in medication administration and provide immediate support for health concerns.

Key Responsibilities:

• Adhere to Wellness Coordination program standards.
• Develop and monitor Wellness Support plans.
• Conduct assessments and complete quarterly reports.
• Assist with medical appointments and maintain accurate medical records.
• Provide direct care and first aid.
• Teach training classes and maintain communication with clients and families.

Requirements:

• Licensed Practical Nurse with CPR, TB, First Aid, and CPI certifications.
• Excellent communication and critical thinking skills.
• Valid Indiana driver’s license and insurability.
