Hospice Nurse with $5000 Sign-On Bonus

Job Description

We are seeking a full-time Registered Nurse to join our expanding clinical team as a Case Manager in a community-based hospice and palliative care organization. This role involves local travel to provide care in patients’ homes and residential facilities in Greater Boston and parts of Metrowest. A $5000 sign-on bonus is offered.

Job Responsibilities:

• Develop and manage comprehensive care plans in collaboration with the interdisciplinary team.
• Conduct clinical assessments of patients’ needs.
• Implement and monitor patient care, focusing on symptom relief.
• Educate patients, families, and healthcare professionals about hospice care and end-of-life processes.

Qualifications:

• Must be a Licensed Registered Nurse in Massachusetts.
• Understanding of end-of-life issues is essential.
• Experience in community health or hospice preferred, but training is provided.
• Strong assessment, organizational, and communication skills are required.
