We are seeking a dedicated Resident Care Manager (RN Case Manager) for our 5-star Skilled Nursing Facility in Davis, California, which serves 37 residents. This role involves supporting nursing staff, collaborating with healthcare teams, and managing patient care through assessments, education, and care plan updates.

Qualifications:

Schedule: 11:00 AM – 7:30 PM; 3 days on, 3 days off

Wages: $45.00 – $55.00 per hour, DOE
