Job Description

Immediate openings for Home Health RN Case Managers in the Southwest Metro Denver area. This full-time, per visit position offers a flexible schedule, excellent benefits, and a sign-on bonus of $5,000. Estimated annual salary ranges from $80,000 to $100,000, with mileage reimbursement at 62.5 cents per mile.

Key Responsibilities:

• Provide patient care and case management under physician direction.
• Develop and modify individualized care plans.
• Collaborate with healthcare teams and educate patients and families.
• Supervise C.N.A.s and LPNs.

Qualifications:

• Graduate of an accredited nursing program; BSN preferred.
• 1-2 years of experience preferred.
• Current Colorado RN license and CPR certification required.
