Job Description

As a Home Health RN, you will serve as a patient advocate, managing the care of Home Healthcare patients and ensuring adherence to quality guidelines for optimal clinical outcomes. Your responsibilities include:

• Coordinating and scheduling care delivery with a multidisciplinary team.
• Ensuring the patient care plan is complete, timely, and appropriate.
• Facilitating communication among team members and community resources.
• Maintaining accurate and timely documentation.
• Precepting new nurses.
