MDS Coordinator, RN – $10,000 Sign-on Bonus

Job Description

Job Summary: The MDS Coordinator at Sheridan Memorial Hospital is responsible for ensuring timely and accurate assessments and interdisciplinary care plans for elderly residents, adhering to Federal and State guidelines. This role includes identifying resident needs, communicating with families, and serving as the Infection Preventionist.

Essential Functions:

• Complete MDS assessments and care plans for residents.
• Monitor and advise on the completion of MDSs by other disciplines.
• Conduct care plan conferences and maintain updated care plans.
• Facilitate interdisciplinary team activities and ensure compliance with documentation.
• Participate in management meetings and promote ongoing education.

Qualifications:

• Bachelor’s degree in nursing and current RN license required.
• Minimum two years in a supervisory role and one year in skilled nursing.
• Experience with MDS 3.0 and Infection Control preferred.

Full-time position with a $10,000 sign-on bonus.
