This position involves providing direct medical support and education to individuals in assigned programs, advocating for their health needs. Responsibilities include:

  • Assessing and coordinating healthcare services for participants.
  • Transporting individuals to medical appointments as needed.
  • Documenting nursing interventions and maintaining health records.
  • Reviewing Individual Support Plans (ISPs) and training staff on medication policies.
  • Communicating medical issues to relevant staff and attending meetings for updates.

    • Qualifications include a current MA LPN or RN license, one year of experience with individuals with MR/DD, and a valid driver’s license.
