RN/MDS Coordinator -1st shift

Job Description

We are seeking a qualified MDS Nurse Coordinator to join our team. The ideal candidate must be a licensed RN with MDS experience and will be responsible for:

• Timely completion of assessments and interdisciplinary care plans in compliance with Federal and State guidelines.
• Identifying resident needs and communicating with families.
• Monitoring PDPM completion and ensuring accurate documentation.
• Managing the Restorative program and Quality Measure Program.

Qualifications include 2 years of MDS and care plan experience, knowledge of Medicare MDS and PDPM, and experience in restorative programming. We offer competitive salary, benefits, and a positive work environment.
