This role involves coordinating patient wellness and chronic illness activities, with success measured by patient outcome performance metrics. The Clinician II also provides clinical leadership, employing critical thinking and problem-solving skills to enhance care delivery.
Minimum Qualifications:
- Education: RN required; BSN preferred.
- Licenses: NM RN license and BLS certification required.
- Skills: Effective communication, teaching techniques, and educational material coordination.
- Experience: One year in a clinical nursing role preferred.
Supervision: Responsible to CSVCG Dyad, supervising medical assistants and ensuring clinical staff competency.
Physical Requirements: Full range of motion, ability to lift up to 50 pounds, and exposure to clinic-related conditions.
