Job Description

We are seeking an experienced Advanced Practice Provider (NP/PA) for full-time onsite medical care in skilled nursing facilities in the Manitowoc area. This role focuses on acute and chronic care management for adult and geriatric patients, collaborating with physicians to enhance patient outcomes.

• Deliver primary care and develop personalized treatment plans.
• Establish diagnoses, prescribe medications, and perform procedures.
• Ensure thorough documentation and coordinate with specialty providers.
• Foster relationships with patients, families, and staff.
