Job Description

We are seeking a talented MDS Nurse, with a preference for RNs, to join our caring team. The MDS Nurse will oversee the completion of MDS/RAI/care plans and ensure safe resident care from admission to discharge. Key responsibilities include MDS assessments, care planning, and serving as a clinical resource for the nursing team. Candidates must have a strong understanding of MDS, Medicare, and care planning processes.

• Job Type: Full-time
• Schedule: 8-hour shifts, Monday to Friday
• Location: Canton, OH (in-person)
• Experience: 1 year in MDS required
• License: RN required
