Job Description

Position Title: Dietitian – Certified Diabetic Educator (CDCES)

Location: Upland Hills Health – Dodgeville Hospital Campus

Role & Department: Clinical Nutrition Department

Hours & Shift: Full-time, Monday to Friday, 8 a.m. to 4:30 p.m.

Role Responsibilities:

• Coordinate the Diabetes Self Management Education (DSME) Program.
• Assess patient needs and plan comprehensive care.
• Provide education and support to patients and families.
• Conduct nutritional assessments and counseling.
• Collaborate with healthcare team members.

Qualifications:

• Bachelor’s degree in Dietetics.
• Registered Dietitian and Certified Diabetes Educator (CDCES) or eligible.
• Knowledge of chronic disease care standards.
