Key responsibilities:
Provide home-based care for patients post-discharge
Collaborate with physicians and oversee CNAs
Conduct assessments and document patient progress
Educate patients and families on care plans
Coordinate discharge and ensure care goals are met
Requirements:
Active RN license in Virginia
1 year of nursing experience, preferably in home healthcare
CPR certification and knowledge of home health regulations
Strong communication skills and compassion
