Job Description

We are seeking a compassionate RN Case Manager to join our team, supporting Meridian and surrounding counties. This full-time position offers a Monday-Friday schedule from 8 AM to 5 PM.

Responsibilities:

• Manage patient and family caseloads through all nursing process phases
• Assess patient needs and coordinate care with the interdisciplinary team
• Conduct regular visits and maintain accurate patient records
• Supervise Home Health Aide care delivery

Qualifications:

• Graduate from an accredited nursing program
• Current RN license and CPR/First Aid certification
• Valid driver’s license and insurance
