Job Description

The Licensed Practical Nurse (LPN/LPT/LVN) provides direct and indirect care to clients in their homes, following organizational policies. Key responsibilities include:

• Delivering skilled nursing care per physician’s orders.
• Documenting care, health status, and communicating with families and physicians.
• Administering medications and conducting health teaching.
• Completing assessments and ensuring compliance with regulations.
• Training new employees and participating in staff meetings.

Qualifications include a nursing degree, medication administration experience, and a current state license. Home health care experience is preferred. Salary is $31.00/hour.
