Pediatric Home Team (Transitional) Nurse

Job Description

Position Overview:

• Assess post-acute needs and ensure continuity of care.
• Focus on medical treatment for infants, children, and adolescents in home settings.
• Administer medications, conduct assessments, and educate families.
• Provide interim care and training for field nurses.

Qualifications:

• Active Texas RN or LPN license and CPR certification.
• Trach/vent and home care experience preferred.
• Strong communication skills and adaptability.
