Registered Nurse (RN), Home Health – Stockton

Job Description

Position Summary: The Registered Nurse (RN) will collaborate with patients, families, and healthcare providers to develop and implement personalized care plans, monitor patient conditions, and provide necessary nursing care for chronic illnesses and injuries.

Responsibilities:

• Conduct comprehensive home assessments to identify patient needs.
• Develop and document care plans in collaboration with healthcare teams.
• Evaluate the need for additional services and equipment.
• Make safety recommendations for the home environment.
• Initiate therapeutic interventions and provide patient education.
• Maintain high standards of clinical care and update patient records.

Qualifications:

• Graduation from an accredited nursing program and current RN license.
• Minimum one year of clinical nursing experience; home health experience preferred.
• Valid driver’s license and vehicle required.
• Strong interpersonal and communication skills.
