Job Description
> REF:#995fd98c79c2e91bSchedule: Monday – Friday, 8am – 4:30pm, with part-time and full-time options (2-3 or 4-5 days/week) including on-call rotation.
Scope: Provide support to patients, families, and caregivers by triaging calls and making weekend visits. Plan, coordinate, and document care for terminally ill patients from admission to discharge, ensuring compliance with regulations and organizational policies.
Key Responsibilities:
- Case Management: Conduct assessments, assist with care plans, and supervise home health aides.
- Palliative Care: Manage pain and symptoms, educate families, and ensure patient safety.
- IDG Participation: Collaborate with the interdisciplinary team and supervise aides.
- Bereavement Care: Support families through bereavement plans and community events.
- Other Duties: Additional responsibilities as assigned.
Education and Experience Required: RN licensure in Minnesota and North Dakota, CPR certification, valid drivers license, computer skills, and ability to communicate effectively in English.
