We are seeking a Physical Therapist to join our home care team. Key responsibilities include:
Greeting customers warmly and creating a positive first impression.
Managing a patient caseload with expertise in functional disorders, including ADL deficits and balance issues.
Providing assessments, treatments, and interventions for performance dysfunction and pain control.
Consulting with patients, families, and caregivers regarding treatment plans.
Instructing on the use of assistive devices and evaluating treatment outcomes.
Documenting care plans and maintaining patient records per agency policy.
Qualifications include a Master’s Degree in Physical Therapy, current state certification, and 3+ years of experience in home health or rehabilitation settings.
