Licensed Clinical Social Worker / LCSW

Job Description

We are seeking an experienced LCSW to provide counseling services that support patients’ physical, mental, cognitive, and emotional recovery. You will deliver therapeutic interventions and resource recommendations to help patients achieve their personal and independent living goals in an integrated setting.

Responsibilities Include:

• Conducting comprehensive evaluations of patients and families
• Setting measurable objectives with the rehab team
• Assisting health team members with social and emotional factors
• Counseling patients and families on coping strategies
• Coordinating care and identifying community resources

Qualifications:

• Master’s Degree in Clinical Social Work (LCSW)
• Current LCSW licensure
• Minimum one year of rehabilitation experience
• CPR certification
• Ability to lift up to 50 pounds
