Job Description

The Licensed Practical Nurse (LPN/LPT/LVN) provides direct and indirect care to clients in their homes, focusing on health maintenance and disease prevention. Key responsibilities include:

• Administering skilled nursing care per physician’s orders.
• Documenting care provided and communicating with families and healthcare professionals.
• Conducting assessments and providing health education to clients.
• Ensuring compliance with regulatory standards and maintaining patient confidentiality.
• Training new employees and participating in staff meetings.

Qualifications include a nursing degree, medication administration experience, and a current state license. Home health care experience is preferred. Salary is $29.00/hour.
