We are seeking a dedicated RN Case Manager in Lancaster, PA, to provide essential one-on-one care in patients’ homes while supporting their families. This role offers flexibility, autonomy, and the chance to enhance your nursing skills.

Key Responsibilities:

• Plan, direct, and deliver safe patient care.
• Collaborate with the Interdisciplinary Team to meet individual patient needs.

Schedule: Mon-Fri, 8 AM-4:30 PM, with on-call rotation and holiday coverage.

Qualifications:

• Graduate of an accredited nursing school with current licensure.
• 1 year nursing experience required; 3+ years preferred.
