Location: Pewaukee
Schedule: M-F, 8:00 AM – 4:30 PM, no weekends, with occasional on-call duties.
Compensation: $10,000 sign-on bonus with a 2-year commitment.
About the Role: The RN Case Manager will coordinate and oversee patient care in a home hospice setting, working with an interdisciplinary team to ensure high-quality, cost-effective care. Ideal candidates should be independent, critical thinkers with strong communication skills and experience in pre-acute and post-acute care.
Requirements:
- Associates Degree in Nursing (Bachelors preferred)
- Current WI RN license
- Valid WI drivers license
- Current BLS certification
- Clinical experience preferred
- EPIC software experience preferred
