Start of Care Nurse – Home Health Care

Job Description

This position involves providing skilled nursing care according to the client’s treatment plan, including health evaluations, service monitoring, and direct nursing care. Responsibilities include assessing patient needs, developing and updating care plans, administering medications, and supervising LPNs and aides. The role requires effective communication with patients, families, and healthcare teams, as well as adherence to regulations. Candidates should have an RN license, CPR certification, and preferably one year of home health experience. Full-time benefits include health insurance, 401k, paid time off, and flexible scheduling.
