> REF:#8dc6fb017b15932fOverview: The registered nurse is responsible for planning, organizing, and directing hospice care, focusing on community health education. They utilize community resources to meet the needs of patients and families in their homes.
Responsibilities:
- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Develop and initiate care plans, regularly re-evaluating and revising as necessary.
- Administer medications and treatments as prescribed, providing health care instructions.
- Coordinate care as a Case Manager and maintain accurate clinical documentation.
- Communicate with physicians and community health personnel to ensure continuity of care.
- Supervise ancillary personnel and participate in ongoing professional development.
Qualifications: Must be a graduate of an accredited nursing program with current RN licensure and CPR certification. A bachelor’s degree and hospice experience are preferred.
