Position: Occupational Therapist
Location: Tualatin, OR

Minimum Qualifications:
– Oregon Occupational Therapist license
– Degree from an accredited Occupational Therapy program
– CPR Certification
– National Provider Identifier (NPI) upon hire

Preferred Qualifications:
– Home Health experience
– Strong organizational skills

Responsibilities:
– Develop treatment plans per physician orders
– Complete clinical and progress notes
– Educate patients and caregivers on treatment goals
